Partial Hospitalization Program

The Partial Hospitalization Program (PHP) provides over 34 hours of specialized treatment per week, including one individual nutrition and one individual therapy session. Five days a week, clients are checked for orthostatic vitals, attend almost four hours of group therapy and are supported through two meals and one snack; see schedule for more information. A weekly internal medicine/medication management follow-up is required. Experiential meal outings and grocery shopping are a regular part of the program.

Intensive Outpatient Program

The Intensive Outpatient Program (IOP) provides over 13 hours of specialized treatment per week, including one individual nutrition and one individual therapy session. Three days a week, clients attend over two hours of group therapy and are supported through one meal and one snack; see schedule for more information. A psychiatric follow-up is required every other week. Experiential meal outings and grocery shopping are a regular part of the program.

Outpatient Program

The Outpatient Program (OP) is tailored to meet the client’s individual needs. SNWC recommends weekly individual nutrition and individual therapy sessions, as well as psychiatric or internal medicine follow ups based on medical necessity.

Admission Process

Admission to our Partial Hospitalization Program (PHP) or our Intensive Outpatient Program (IOP) include individual meetings with the psychiatrist/ internal medicine physician, the dietitian and the therapist in which the client participates in comprehensive initial assessments with each provider. During this process, the client completes psychological assessments to establish baselines that at discharge will help the client and the clinical team determine which goals have been met and where growth opportunities still exist. This informs us how to effectively support our clients in resuming their normal activities of daily living, including work, school, housing and implementing a continuing care plan. Ongoing assessment of treatment effectiveness allows us to continuously improve the care we offer within all of our programs.

Insurance: We are In-Network with most major insurance providers. We work collaboratively with any Out-of-Network providers to negotiate coverage for our clients.

Living Options: While attending any of our programs, clients may live at home, at a local hotel or in transitional living. We work with some hotels in the area to provide our clients a discounted rate for extended stay.

Additional Services: Clients can request additional IOP days, individual, couple, family or other specialized sessions for an additional fee.
